Once in a great while ich will show up about a week after the fish goes into hypo treatment, but usually it won't be seen beyond that point. Are you using a refractometer to measure your salinity or a cheap hydrometer? I've seen hydrometers read 0.005 too low, which means that your tank may be at 1.014, in which case ich could still survive.
