Default Fish are dying

I have a 55 gallon tank with a yellow tang, 5 green chromis, and 2 clown fish. I also have a chocolate chip star fish and a red serpent star. I had a cleaner shrimp, but one morning I woke up and he was gone. Then a few days later a chromis was gone. I woke up yesterday to find the chocolate chip start on top of the clownfish eating both of them. Today I woke up to find that my yellow tang's fins were shredded and her face was peeling and bleeding. I am not sure what is happening in my tank.

Do you have live rock in the tank? Might have picked up a hitch hiker (mantis shrimp) that is attacking your fish.
