Default HELP constant bacterial problems

my tank get a bacterial infection jst about every 3 weeks. can anyone shed on light on this??

Ok if it is a disese like ick raise the temp to 85 turn of the lights and I suggest melafix to put into the water let me know what happens

How can you tell if your tank has a bacterial infection?
