Default loss of color in head?

What conditions cause a fish to lose color in thier head? I am waiting for my refractmeter (srry about spelling.) to get here, they don't sell them here. All water test perfect. i have 3 hydrometers all are cleaned but i have 3 diff. readings. I used the new one and it said the salt was low. iv'e ajusted it. trusting the newest inant ocean brand. please let me kmow. Body clear of and thing. Please help. ty

I added more salt and a power head and he is great now. I forgot of this post, srry. I got him better in 2-3 hours and was so relieved. I forgot i wrote this. any way, food = Brime shrimp, tubeflex worms, His fav tropical flakes, and freeze dries baby shrimp ( cleaner crab loves his own kind.) he was swiming lslow and faceing down. All color has returned. My refractmeter should have been deliverd already. I have 3 hydrometers and dispite soaking them in vinegar, they all read diff. they say my tap water has 1.008.. when i test salt water mix with 1/2 cup per gal. It reads 1.032
fish= I have a true perc. clown fish

i have 2 dif. puffers. in dif. tanks and in the 12g theres a cleaner shrimp, scarlet leg hermit crab and the clown fish
