Default Red Bali Star

So i have had my first fatality. I have been running my system for about a month. It was a complete transfer from someone else (water and substrate included)

I just bought a red Bali Star 3days ago and to my surprise I came home from work today to find his poor body mangled among my live rock. I only have a couple damsels and a few snails running around. The only thing I can think of was that he might have been offended by my water change yesterday. I am at a loss. My water lvls had been good up until today. Nitrate was on the rise. So I regretfully had to remove my Bali. Any suggestions. I have been looking but no real results anywhere.
